Finding Specialized Living Accommodations
Types of living situations may include Independent Living Communities, Assisted Living Communities, Skilled Communities, Board and Care Homes, and Alzheimer's/ Dementia Communities.

Comprehensive Assessments

We complete a non-medical assessment, to determine the needs and desires of the Senior. This includes the care level needed, the "ideal" location, and the financial situation. We also obtain the medical information, to help in finding the proper community. (A medical release is signed by either the Senior or the POA)

Determine Financial Assistance

We help explore all financial assistance that may be available, such as Long Term Care Policies, VA Aid and Attendance Benefits, and Savings. We also help determine if Medicaid is in the near future.

Set Up Tours for Seniors and Family Members

Once all of the information is obtained, we set tours with the family and communities. We will take the family to the communities or meet them there (depending on the situation and convenience for the family). After the decision is reached as to the community that is the best fit, an assessment is completed by the community. We obtain the Physician's orders, and usually follow the Senior into the new community with Occupational Therapy and Physical Therapy (OT/PT). The OT/PT is generally covered by Medicare,or an insurance company, at no cost to the Senior. It is set up through an "In-Home" health company.

Utilize an Extensive Network of Professionals

We have a extensive Network of professionals assisting with Elder Care legal advice, financial planning, downsizing, estate sales, moving, and space planning for the new living area.
